Sciatica: Symptoms, Non-Invasive Treatment Options

What is sciatica?

Sciatica is pain that occurs when the sciatic nerve is irritated, compressed, or inflamed. This is the largest nerve in the body, starting in the lower back, passing through the buttocks, running down the back of the thigh and calf, and possibly extending to the foot, heel, or toes.

In medicine, sciatica is also referred to as lumbosacral radiculopathy, sciatic neuralgia, or nerve root pathology. However, “sciatica” remains the most common term, describing pain that originates in the lower back and radiates down the leg along the path of the sciatic nerve.

Signs and symptoms of sciatica

The sciatic nerve has two extremely important functions: controlling movement and transmitting sensory signals.

Motor function: The sciatic nerve sends signals from the brain to the muscle groups in the thigh, lower leg, and foot, helping us perform familiar movements such as walking, running, standing up, sitting down, raising the heel, or flexing and extending the toes.

Sensory function: The sciatic nerve carries signals from the leg back to the brain, helping the body sense heat, cold, pain, numbness, or touch along the length of the leg.

Therefore, when the sciatic nerve is compressed, patients often develop two clear groups of symptoms: sensory disturbances and motor disturbances.

Signs of sensory function impairment

When the sciatic nerve is compressed, sensory signals sent to the brain may be disrupted. Instead of normally sensing touch, heat, cold, or pain, the nerve may send “false alarm” signals to the brain, causing the patient to feel unusual pain, numbness, or tingling.

  • Burning pain along the path of the sciatic nerve: The pain often feels like an electric shock, stabbing pain, or burning. The patient does not only feel pain in the lower back, but the pain runs in a clear line from the lower back, through the buttocks, down the back of the thigh, calf, sole of the foot, or toes.

  • Numbness and tingling (paresthesia): A tingling leg is often described like sitting too long and poor blood circulation, or like thousands of ants crawling under the skin. The numb area often appears on the top of the foot, heel, big toe, or little toe, depending on the location of the compressed nerve root.

  • Reduced or lost sensation: The patient cannot clearly feel heat, cold, or pain in a patch of skin on the leg. 

Signs of motor function impairment

When the motor function of the sciatic nerve is affected, signals from the brain to the muscle groups in the leg are interrupted. At this point, the patient may have difficulty lifting the front of the foot or the toes off the ground while walking. In addition, the painful leg may become noticeably weaker. The patient may have trouble performing simple movements such as raising the heel, standing on the heel, flexing and extending the toes, or standing up from a chair without using the armrests.

Causes of sciatica

About 90% of sciatica cases are associated with herniated discs. In addition to herniated discs, sciatica can also be caused by spinal bone spurs. When bone spurs form at the edge of the vertebrae, the sciatic nerve can become irritated and cause sharp pain. Less commonly, tumors or abnormalities in the spinal canal can also put pressure on the nerve and cause pain.

Factors that increase the risk of sciatica include:

  • Age: People between 20 and 50 years old, especially older adults, have a higher risk of herniated discs. As we get older, the discs and spinal joints become more prone to wear and tear. This creates conditions that allow sciatic nerve compression or other spinal problems to develop.

  • Obesity: Excess weight places more load on the spine, especially the lower back. Prolonged pressure can overload the discs, increasing the risk of back pain and sciatica.

  • Work demands: People who frequently bend forward, twist their back, lift heavy objects, or drive long distances are more likely to place repeated stress on the lumbar spine. 

  • Prolonged sitting and lack of exercise: People who sit for long periods or are physically inactive generally have a higher risk of herniated discs than those who stay active. Long sitting increases pressure on the lower back, making the spine more unstable.

  • Diabetes: Diabetes causes high blood sugar levels, which can damage blood vessels and nerve fibers. Therefore, if you have diabetes and experience symptoms such as numbness, tingling, or pain radiating down the leg, you should seek medical attention early.

Who is at the highest risk of sciatica?

According to statistics, about 5-10% of people with lower back pain may develop sciatica. Notably, lower back pain itself is very common, with as many as 49-70% of the population experiencing it at least once in their lifetime.

In addition to factors related to lower back pain, certain people are also at higher risk of sciatica:

  • Middle-aged and older adults: Over time, disc degeneration, bone spurs, or spinal stenosis can increase the risk of nerve root compression.

  • Tall individuals: A longer spine may be subject to greater mechanical stress, especially when combined with poor posture, prolonged sitting, or heavy labor.

  • People who are frequently under mental stress: Prolonged stress can increase muscle tension, causing the lower back, buttock muscles, and muscles around the spine to become tight, making pain radiating down the leg more noticeable.

  • Smokers: Smoking can affect blood circulation that nourishes the discs, increasing the risk of degeneration and spinal structural damage.

  • People whose jobs require long sitting or frequent vibration: Truck drivers, machine operators, and others are more likely to experience repeated pressure on the lumbar spine, increasing the risk of sciatic nerve pain.

Can sciatica be cured?

The good news is that sciatica can improve well with the right treatment approach. Regardless of whether the cause is a herniated disc, bone spurs, or lumbar nerve root compression, about 90% of sciatica cases recover without surgery.

However, this does not mean patients can ignore the pain or wait for the symptoms to go away on their own. If you or a family member has signs of sciatica, early evaluation is essential. Prolonged sciatica can cause more serious nerve damage. Patients may develop muscle weakness, unsteady walking, difficulty lifting the front of the foot while walking, or a tendency to trip and fall. In some rare cases, patients may live with permanent nerve damage.

The sciatic nerve lies deep inside the body, surrounded by vertebrae, discs, ligaments, and muscle groups. Therefore, imaging studies are needed to clearly observe the internal structures and assess the degree of injury.

Common diagnostic techniques include:

  • X-ray or CT scan of the spine: Helps assess bone structure, detect spondylolisthesis, narrowed disc spaces, degeneration, bone spurs, or abnormalities that narrow the space around the nerve roots.

  • MRI: Provides more detailed images of the discs, nerve roots, and ligaments. This method is often ordered when the doctor suspects a herniated disc, spinal stenosis, or severe nerve compression.

  • EMG or myelography: May be indicated in complex cases, such as patients with diabetes or suspected peripheral nerve damage.…

How can sciatica be treated?

Depending on the degree of nerve compression and the cause of pain, sciatica can be treated with different methods, from home care, medication, physical therapy, and Chiropractic adjustments.

Home care for mild symptoms

In the early stage, if the pain is not too severe and there is no loss of sensation or motor disturbance, the doctor may guide the patient on how to relieve sciatica at home using the following methods:

  • Cold and hot compresses: During the first few days, cold compresses may help reduce pain and inflammation around the lower back. After that, hot compresses help relax the muscles, improve blood circulation, and reduce tightness. Each application should last about 15-20 minutes.

  • Over-the-counter medications: Some over-the-counter pain relievers or anti-inflammatory drugs may help temporarily ease the pain. However, patients should not overuse them for long periods, especially if they have stomach, liver, or kidney conditions, or are taking other medications.

  • Gentle movement and stretching: Prolonged complete rest can make the back muscles, buttock muscles, and core muscles weaker. Instead, patients should maintain light activity, short walks, and appropriate stretches for the lower back and buttocks to reduce pressure on the sciatic nerve.

If symptoms do not improve after a few weeks, the patient should seek medical evaluation promptly to identify the exact cause and receive an appropriate treatment plan.
